By electronically completing this form, I acknowledge the following statement of consent and release of liability. I am 18 years or older and understand I remain fully responsible for the actions and conduct of myself and my dependent(s). I understand that a part of this event will take place away from the church grounds and that my dependent(s) and I registered here will be with church members on the stated dates. I further consent to the conditions stated above on participation in this event. Release and Waiver of Liability: In consideration of myself and my dependents being allowed to participate in this conference, I hereby agree on behalf of myself and my dependents, to release Rhema Gospel Church, their employees, agents, and representatives, including volunteers (collectively “Releasees”) from any and all claims, including negligence, which may be asserted by me or my dependent(s), or on behalf of my dependent(s), arising from or relating to my or my dependents' participation in the conference Indemnification: In the event this release on behalf of myself and/or my dependent(s) is/are held to be invalid or unenforceable, I hereby agree to indemnify and hold harmless Releasees from any and all claims, including negligence, which may be asserted by me or my dependent(s), or on behalf of me or my dependent(s), arising from or relating to my or my dependents' participation in the conference. This release or indemnification does not apply to claims for intentional misconduct or gross negligence; nor does this release or indemnification apply to the extent of commercial insurance coverage for any claim, but this Release or Indemnification shall apply to the extent of any self-insurance or deductible applicable to any claim. Medical Treatment: I consent to receive medical treatment deemed necessary by the event organizers in case of injury, accident, or illness during the Conference. I understand that I am responsible for any and all medical expenses incurred as a result of my participation in this event. Legal Capacity and Consent: I certify that I am at least 18 years of age and legally competent to sign this Release of Liability. If under 18 years of age, my parent or legal guardian must sign on my behalf.
